REGISTRATION FORMS
Thank you for taking the time to complete your registration forms online. This helps us dedicate more time to you when you’re in the office. Plus, it ensures we are prepared to answer any questions you may have. Filling out registration paperwork may seem monotonous, but it’s the first step to the rest of your life.
- Please complete the forms linked below in advance of your appointment.
- If you have recent x-rays or CT scans, please bring them with you or have them sent to our office!
- After any needed tests have been completed and you’ve met with the doctor, plan to review your custom treatment plan, which will include all expected costs.
- Please note, we bill insurance as a courtesy, but it is ultimately your responsibility to maintain coverage and let us know about any changes.
PRE -OP INSTRUCTIONS
Our doctor will review the entire treatment plan with you, including anything you’ll need to do to prepare and steps you’ll need to take for a speedy recovery. However, the following general information may give you an idea of what to expect. As always, don’t hesitate to contact us to review any questions you have.
If your treatment plan includes sedation, please prepare accordingly. This includes:
- Don’t have anything to eat or drink for 8 hours prior to your surgery.
- Come with a driver over 18 to accompany you home.
- Wear a short sleeve shirt for IV placement.
- Wear shoes that stay on, to reduce any potential instability after your procedure.
